SEC Form 15-12G
A certification of termination of registration of a class of security under Section 12(g), or notice of suspension of duty to file reports pursuant to Section 13 and 15(d) of the 1934 Securities Exchange Act Section 12(g).

Section 12(g) of the Securities Exchange Act concerns exceptions to the normal filing requirements for registering a security with the SEC. The data provided to the SEC would be regarding why the security would be exempt from the 1934 act. Some examples would be mutual funds, some insurance company products, co-ops, etc.
